EBILL/ECONSENT FORMS

AN IMPORTANT BILLING MESSAGE FROM THE OFFICE:

Please be sure that if you want to sign up for e-billing (to receive your bill via email) and to remove the $1.50 paper bill fee for the next bill, YOU MUST HAVE AN ELECTRONIC CONSENT FORM ON FILE. If you haven’t filled one out yet, please click here to download the form. Please return it to the office ASAP, the Deadline is April 30th. You can still turn in your form at anytime, but anything received after April 30th may not provide enough time to process your request for the upcoming bill cycle, and you will have to wait until the next bill cycle for ebilling.
