• ENROLL IN E-BILLING, OPT OUT OF PAPER BILLING, and REMOVE THE $1.50 BIMONTHLY FEE (MEMBERS WHO RECEIVE A PAPER BILL ARE ASSESSED A $1.50 PAPER BILLING FEE), and/or

• RECEIVE CERTAIN REQUIRED COMMUNICATIONS VIA EMAIL (SEE FORM FOR MORE IMPORTANT INFO).
